You must fill up the form to request for the card which will be delivered to you on your mailing addresses anywhere in Pakistan. Residents outside Pakistan can authorize someone in the country to collect the card on their behalf.
Once your information is received you will receive the card for free.

Profile Picture (high resolution with a plain, professional backdrop in .jpeg or .png format. Maximum photo size should be 300 x 400 pixels in width and height.)
